Join PAA as a Fellow or Associate  Member

The three ways you can join PAA as a Fellow Member are described below. Once we have your PAA Fellow Membership Application Form and annual dues, we will provide you with your user name and password for the PAA Members Only section via regular mail or email.

To join, you may

1. Contact our VP of Membership, MaryLou Astorino, Au.D., by letter at ENT Associates of Johnstown, 321 Main Street, Suite 4F, Johnstown, PA 15901, by calling (814) 536-0401, or by email. Tell MaryLou that you would like to join PAA as a Fellow or Associate Member and provide her with your name, address, phone number, and email address. MaryLou will send you a PAA Fellow or Associate Membership Application Form and instructions.

2. Proceed to download a PAA Fellow/Associate Membership Application Form in Adobe Acrobat Reader PDF format, then print it out, fill it in, and mail it in with a check for your dues.
