Join PAA as a Student Member
To join PAA or renew as a Student Member, you may either
1. Contact our VP of Membership, MaryLou Astorino, Au.D., Au.D., by letter at ENT Associates of Johnstown,
348 Budfield Street, Johnstown, PA 15904, by
calling (814) 536-0401, or by email.
Tell MaryLou that you would like to join PAA as a Student Member and provide her
with your name, address, phone number, and email address. MaryLou will send you
a PAA Membership Application Form and instructions.
or
2. Download a PAA Student Membership Application Form, print it out, fill it in, have your supervisor sign it
for verification, and mail it to the address shown on the form.
